PCMH Training and Technical Assistance

The goal of PROMPT is to support Rural Health Clinics (RHCs) and rural provider practices in transforming rural clinic practices to meet the ever-changing demands within the healthcare landscape, improve clinical quality data capture and reporting to better meet the needs of funders, payors, and patients, and ultimately improve quality performance and patient care in rural Missouri. PCMH provides the foundation RHCs and rural provider practices need to transform clinic practices to meet high-quality standards and performance outcomes, including:

  • Team-Based Care – reduce fragmentation in care, improve care coordination, and close care gaps.
  • Improved Performance and Quality Outcomes – improve Clinical Quality Measures (CQMs) and reduce healthcare costs.
  • Payor Alignment – ability to participate in Value Based Payment (VBP) initiatives.
  • Additional Revenue Stream – prepare RHC’s to participate in federal/state Quality Improvement (QI) initiatives, e.g., Merit-based Incentive Payment System (MIPS), Primary Care Health Home (PCHH), QI Performance, etc.
  • Quintuple Aim Alignment – demonstrate RHC alignment with Quintuple Aim measures.
  • Access to Care and Care Coordination – increase same day access to care to improve patient access to quality healthcare and improve care coordination to reduce emergency department (ED) utilization and hospital readmission.
  • Population Health Management – identify barriers and provide resources/support to reduce Social Drivers of Health (SDOH) barriers to improve health outcomes; stratify patient population to address disparities in care.
  • Compliance – achieve federal/state reporting requirements.
  • National Recognition – achieve NCQA PCMH recognition, often viewed as the “gold” standard of care.
